                      Cost:     Golf: $125 per golfer ($150 if registering after May 15)
                                 (includes golf w/ carts, access to range, lunch)

                                            Lunch Only: $50 per person
                                            (includes lunch and auction number)

                                            Hole Sponsor: $100 per hole
                                            (company name will be displayed on a hole)
